By now, we have already connected and discussed, what's next to come. All of your questions and concerns have been answered and we are ready to proceed. Lucky O'Neill Dispatch & Logistics, LLC will send our onboarding packet immediately to you. This packet will contain the following: A Carrier Profile, Driver Questionnaire, Dispatcher-Carrier Agreement, Limited Power of Attorney (POA) and Payment Authorization From. These forms are to be read and completed by you, our Client/Carrier/Driver. ~ Onto step 3...Somewhere over the rainbow towards that pot of gold! ~

Client/Carrier/Driver to Lucky O'Neill Dispatch & Logistics, LLC.
After the onboarding packet has been completed, you will send back all documents within the packet to Lucky O'Neill Dispatch & Logistics, LLC. Please be sure all required information is filled out, signed and dated where applicable. (Remember to make or keep copies for your records). In addition to the onboarding packet, you will also send us your Certificate of Authority, Proof of Insurance, copy of CDL, Factoring info (if applicable) and signed W9. Once, we receive the completed onboarding packet and Carrier/Client/Driver requirements, Lucky O'Neill Dispatch & Logistics, LLC will begin to and consistently find the end of the rainbow for you, which leads to that "pot of gold" you've been searching for!
